How to Request Medical Records

To release your medical records, we must have a signed Authorization for Release of Health Information form. See below for the 2 types we currently provide.

• Health Provider to Release Records to FiCAREMed
• FiCAREMed to Release Records to another Health Provider

Medical records can never be released to anyone other than the patient if they are over 13 years of age. This includes child to parent, spouse, vice versa unless we have a current Authorization For The Release Of Medical Records form.
